The following must be completed to ensure eligibility to file the DR-465 application:

Before proceeding, please be aware, for purposes of determining uninhabitability, the application must be
accompanied by supporting documentation, such as Utility Bills, Insurance Information, Contractors’ Statements,
Building Permit Application OR Building Inspection Certificates of Occupancy.

Please be ready to provide at least two of these document types.
